Transitioning from rest to exercise is often initiated in the elderly who have been sedentary for many years. The effects of this transition on cardiopulmonary function have not been fully explored. The purpose of this study was to investigate changes in cardiopulmonary function in the elderly. Ten community-dwelling elderly individuals (mean age 70 years, seven male) underwent a series of incremental cardiopulmonary exercise tests using a cycle ergometer. Following a baseline assessment, subjects were instructed to pedal at 0.5 W/kg until exhaustion. In a second condition, subjects were instructed to pedal at the same rate until they were within 2 W/kg of their baseline power output, at which point they were told to "hit the wall." Power output and cardiopulmonary data were recorded at each time point. The initial peak power output, time to exhaustion, peak oxygen uptake, and minute ventilation were similar across both conditions. However, peak carbon dioxide output, an index of ventilatory efficiency, was significantly lower in the transition condition. This finding indicates that the transition from rest to exercise results in a large increase in the ventilatory response to exercise, which is likely a result of the increased energy demand of exercise.
